How to use it

  1. Print the sheet below and fill in your name, the program or court requiring the hours, your total requirement, and your deadline.
  2. Log each shift the day you serve it — date, organization, and hours. Memory is the number one reason hours get rejected.
  3. Get your supervisor to sign the row before you leave the site. A signature added weeks later is much harder to get.
  4. Keep the running total updated so you always know how many hours you have left.
  5. When you finish, photograph or scan the signed sheet before you hand it in, and ask the organization for a verification letter on letterhead. Most courts and schools want both.

To print: press Ctrl + P (Windows) or ⌘ + P (Mac). The site navigation and these instructions drop away — you get just the sheet, sized for US Letter.
